Full time permeant role 80 hours per fortnight with monthly ADO
$32.47-$44.34 per hour – depending on experience
About the Role
Grampians Health Ballarat is a renowned Aged Care provider, who have been delivering home care support for over 30 years.
In this new and exciting role, as a care partner you will provide care management and coordinate a range of care and services to people who live at home and receive Home Care Package (HCP) or Linkages Package (HACC-PYP) funding.
You will work in a supportive and collaborative team environment, helping current aged care consumers transition to the Support at Home program commencing on the 1st of November 2025.
Key Responsibilities
- Collaborate within a multidisciplinary and cohesive team to achieve positive outcomes for participants.
- Actively involve participant, their carer and family in the care planning, development and review of a participant’s care plan and budget.
- Deliver service planning, coordination and monitoring of services.
- Apply embedding wellness and reablement approaches to increase participant’s overall wellbeing and quality of life.
- Ensure services are delivered are in alignment with the client’s needs, preferences and rights, with respect to choices, dignity, privacy and respect.
About You
You are an experienced aged or disability worker with demonstrated care management capabilities and a commitment to quality care. You are proactive, adaptable, and collaborative, with a good understanding of the complexities involved in aged care service delivery, and the willingness to actively participate in professional development and contribute to improved care.
Skills and Experience
- Diploma or Certificate III/IV in Aged Care, Nursing, Allied Heath Assistance or Community Services.
- Ability to undertake goal planning within consumer directed care framework and wellness and reablement approaches.
- Strong organisational, time management and IT skills.
- A current Victorian Driver's License.
- Previous experience working within an aged care, disability, or community team would be preferred.
