About the Role
The Residential In Reach (RIR) Clinical Nurse Consultant (CNC) works independently within a multidisciplinary team to provide timely, person centred care to residents in public and private aged care communities across the Grampians region. This is a consultancy service to more than 1200 residents of Aged Care Facilities, public and private, in the Grampians Health region. Another 1300 residents across the wider Grampians region are gradually being added to the service as part of the GRHSP RIR Redesign.
This role supports the prevention of unnecessary hospital admissions, facilitates early discharge, and enhances the capacity of aged care staff through expert clinical advice and education. We require a highly skilled CNC to join our small team in providing your expertise. You’ll work closely with Nurse Practitioners, GPs, geriatricians, and emergency departments to deliver safe, coordinated, and high quality care that aligns with residents’ wishes and the Strengthened Aged Care Quality Standards.
Key Responsibilities
- Provide timely nursing assessments and develop treatment plans
- Support aged care homes during infectious disease outbreaks
- Deliver education and capacity building to aged care staff
- Facilitate early discharge and prevent unnecessary admissions
- Liaise with ED, inpatient units, and GPs to coordinate care
- Contribute to service redesign and regional RIR project delivery
Skills & Experience
- AHPRA Registered Nurse with current practising certificate
- Strong clinical assessment and decision-making skills
- Postgraduate qualification in aged care or related field preferred
- Proficient in IV, ECG, IDC management and acute nursing care
- Excellent communication and stakeholder engagement skills
