Nurse Assessor - Complex Geriatrics

About the Role

The Complex Geriatric Clinic (CGC) provides comprehensive assessments for people aged 65+ with complex medical and functional needs. As Nurse Assessor, you’ll be a key member of the CGC team, coordinating and supporting clients through the clinic process while delivering person-centred care.

• Conduct assessments and support care planning for older clients
• Educate and advise clients, families and carers on care needs
• Collaborate with a multidisciplinary team in weekly clinics
• Support integration of care across services and providers

Key Responsibilities

• Complete initial assessments and develop care plans
• Provide education and advice to clients, families and carers
• Support integrated care pathways for older people
• Coordinate and contribute to weekly geriatric clinics

Skills & Experience

• Registered Nurse with current AHPRA registration
• Clinical skills and experience in geriatrics
• Postgraduate study in geriatrics/related field (or working towards)
• Strong understanding of geriatric syndromes and care needs
• Current Driver Licence

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant (ZF4-ZJ4) which ranges from $59.08 - $62.20 per hour.
