About the Role
We are seeking a dedicated Residential in Reach (RIR) Nurse Practitioner to join our team and make a meaningful impact on the care of residents within Care Communities (CC) across the Grampians region. This role involves developing, implementing, and delivering quality assessment and intervention services for elderly residents in their place of residence, preventing unnecessary emergency department presentations. You will also provide valuable consultancy via telehealth platforms across the region.
Key Responsibilities

Provide operational guidance and support to staff, ensuring efficient and effective service delivery in all care settings.
- Develop and implement Acute Management Plans, incorporating both pharmacological and non-pharmacological strategies, adjusting treatments based on clinical assessments and ongoing monitoring.
- Manage palliative and end-of-life care, prioritising patient comfort, dignity, and support for families, in line with the scope of practice.
- Monitor and assess prescribed therapies, reviewing efficacy, potential adverse effects, and drug interactions to optimise patient outcomes.
- Refer patients to appropriate specialist teams (e.g., medical, surgical, mental health) as needed, ensuring comprehensive care coordination and continuity.
- Collaborate with leadership and multidisciplinary teams to enhance service delivery, provide virtual consultations, and ensure strict adherence to infection control protocols across all care environments.
Skills & Experience

Eligible for registration as a Registered Nurse with AHPRA, hold a current Practicing Certificate & Nurse Practitioner endorsement through AHPRA.
- A minimum of five (5) years of post-graduate experience in aged care, palliative care, or dementia-specific care.
- Advanced clinical knowledge in assessing and managing geriatric syndromes, either individually or in combination, including treatment and/or investigation.
- Excellent interpersonal and communication skills, with the ability to collaborate effectively with multidisciplinary teams.
- Proficient in Microsoft Office and other relevant technology platforms for clinical documentation and communication.
- Strong organisational and problem-solving skills, with the ability to manage a caseload effectively.
