An exciting opportunity exists for an innovative, enthusiastic and experienced Grade 2 physiotherapist to influence and develop the multi-disciplinary services and model of care across our established Community Rehabilitation Team – Neurology Stream.
The primary responsibilities include providing high-quality physiotherapy services to a diverse range of clients with neurological conditions. While treatment is mainly conducted at our centre, some community visits may be required.
Key Responsibilities
- Specialist Care: Provide specialist assessment, diagnosis, treatment, intervention, complex discharge planning, and advocacy for clients with conditions such as stroke, acquired brain injury, spinal cord injury, progressive neurological disorders, vestibular conditions, falls management, and other neurological conditions.
- Continuity of Care: Ensure seamless transitions for clients through the healthcare system by coordinating with relevant staff and community agencies, guaranteeing continuity of care for patients and their families.
- Standards Compliance: Meet organizational, allied health, program, and discipline communication and documentation standards.
- Team Collaboration: Actively contribute to ward rounds, case conferences, and family meetings.
- Professional Development: Participate in performance development programs, orientation, mentorship, supervision, quality improvement activities, research projects, and stay updated with new developments through journal reviews, in-services, and external educational opportunities.
Skills and Experience
As the successful applicant you will;
- Be registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency.
- Demonstrate a high level of knowledge and skills within the Neurological Rehabilitation field.
- Demonstrate an understanding of developing others through clinical supervision, mentoring, training and role modelling
- Lead, by example, a culture of recognition and success through the active modelling of the Grampians Health Services values: Collaboration, Compassion, Accountability, Respect and Innovation.
- Demonstrate and understanding of relevant service development activities, including evaluation, monitoring, clinical governance and quality improvement.
- Effectively communicate with colleagues across the multi-disciplinary team and all levels of management.
