Activity Optimisation Lead

About the Role
Grampians Health is seeking an experienced and motivated Activity Optimisation Lead to drive data integrity, reporting excellence and funding optimisation across our Community Aged Care programs. This newly created role will play a key part in supporting Commonwealth Home Support Programme (CHSP), HACC-PYP and Support at Home services by leading data management processes, ensuring compliance with funding requirements, and identifying opportunities to enhance performance and sustainability.
Key Responsibilities
- Act as the central point of contact between Community Aged Care, relevant Community Program Managers and other internal and external stakeholders regarding data management.
- Prepare accurate and timely reports, including identifying and correcting data errors through liaison and report development.
- Identify opportunities to enhance patient and clinical applications to improve data collection and workflow efficiency, including the transition to Support at Home and implementation of electronic medical records.
- Work closely with managers and stakeholders across Community Aged Care, Health Information Services and Digital Services to strengthen reporting.
- Use a clinical lens when problem solving.
Skills & Experience
Applicants with any of the following will be considered:
- Qualified Health Information Manager
- Experienced Data Manager
- Allied Health or Nursing qualification with relevant experience or interest in the area
- Suitable work experience in a relevant field & a skilled problem solver
