Care Coordinator - Emergency

Grampians Health Services (Ballarat campus) invites applications for the position as the Care Coordinator- Emergency Department
The Emergency Department inspires evidenced based best practice leading to quality outcomes. If you would like to be part of this dynamic department and work in an autonomous and flexible environment then this position may be for you. The position will incorporate all aspects of Emergency and community-based care with a major focus on discharge planning, safe referral and connectiveness to community services.
To be successful in this role you will have an interest in learning and development, have well-developed skills in community and emergency care, be able to organise and priorities work commitments in an innovative and self-directed manner and demonstrated effective leadership skills.
Selection criteria includes:
- Registration as a Division One Nurse with AHPRA (desirable)
- Post graduate qualification in relevant area highly desirable
- Excellent communication skills
- This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024. The classification is Registered Nurse Grade 4 (YX11 - YX12) which ranges from $54.21 - $55.83 per hour.
