About the Role

Join Grampians Health Ballarat’s Carer Support Service (CSS) and make a difference for carers across the Central Highlands, Central Grampians, and Wimmera regions. In this 12-month fixed-term Case Manager role, you’ll work in a supportive Mt Helen-based team, helping carers set and achieve their goals through a person-centred approach.

  • Assess carers’ needs and develop tailored action plans
  • Provide information, advice, referrals, and advocacy
  • Engage with carers from diverse backgrounds
  • Work collaboratively with internal teams and community services

 

Key Responsibilities

  • Undertake carer intake, assessment, and care planning
  • Coordinate and review support services
  • Deliver carer education and advocacy
  • Build strong stakeholder and community networks
  • Promote and develop carer services
  • Maintain accurate and timely client records

 

Skills & Experience

  • Tertiary qualification in Allied Health, Nursing, or Human Services
  • Knowledge of carers’ needs, rights, and support services
  • Strong assessment, planning, and coordination skills
  • Proficient in Microsoft Office and databases
  • Current unrestricted driver’s licence
