Clinical Nurse Consultant - Diabetes

Fixed Term Full Time until December 2026 | 40 hours per week with monthly ADO (Part time hours also available)
About the Role
Grampians Health Ballarat is seeking a Diabetes Educator to join the team at our Ballarat Campus. As the Diabetes Educator you will provide safe, effective, person-centred clinical care to people affected by diabetes. The care provided will be based on the best available evidence, incorporating principles of teaching, and learning. You will apply your specialised knowledge and skills to promote optimal health and wellbeing and efficiently manage referrals to ensure our clients and consumers can access the care they need in a timely way.
Key Responsibilities
- Provide specialist diabetes services, including educational services and therapeutic direct and indirect clinical care.
- Work collaboratively to deliver person centred, clinical care to service users.
- Maintain advanced clinical skills and knowledge within clinical speciality, demonstrating strong clinical leadership, and acting as a role model within speciality.
- Act as a clinical advisory resource and develop activities and projects within clinical discipline to participate in and contribute to research.
- Work within the scope of practice of primary discipline and in accordance with the Australian Diabetes Educators (ADEA) Role and Scope of Practice for Credentialled Diabetes Educators in Australia.
- Support departmental and organisational initiatives and activities requiring specialist intervention, including education and training of healthcare professionals across several multidisciplinary teams.
Skills & Experience
- Holds a current Registered Nurse Division 1 registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Postgraduate qualification in Diabetes Education
- Membership with specific speciality association and/or nursing professional body and current ADEA Credentialling.
- Demonstrated ability to consult and collaborate with others
- Proven leadership within the healthcare workforce
This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028.
