This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant (ZF4-ZJ4) which ranges from $59.79 - $62.94 per hour.


About the Role

 

The Diabetes Nurse Educator provides specialised clinical support and education to individuals living with diabetes. This role is centred on empowering patient to confidentiality self-manage their condition through evidence-based practice and individualised care planning. Working collaboratively within a multi-disciplinary team, the Diabetes Nurse Educator delivers high-quality, patient-centred care tailored to diverse needs across the continuum of care.

 

The Diabetes Centre is located in Ballarat, in close proximity to the Grampians Health Ballarat Base Hospital.

 

This position offers a diverse and rewarding scope of practice across multiple clinical settings. Diabetes Nurse Educators provide services within acute and sub-acute sites, as well as delivering outpatient consultations. The team supports a broad range of patient cohorts, including paediatric patients, adults with type 1 and type 2 diabetes, diabetes in pregnancy, and individuals using insulin pump therapy.

 

Services are delivered across multiple specialty clinics including paediatrics, maternity outpatients, adult clinics and young adult clinics. The diabetes education teamwork in close partnership with endocrinologists, paediatricians, medical teams, and allied health professionals to ensure coordinated, comprehensive diabetes management.

 

This is an exciting opportunity to join a dynamic and supportive diabetes team at Grampians Health in a permanent capacity. The position is 3 days per week, with availability required Wednesday to Friday.

 

Key Responsibilities

  • Practice in accordance with the Australian Diabetes Educator Association (ADEA) Standards of Practice, and Standards of Professional Performance for Credentialed Diabetes Educators
  • Provide comprehensive patient education and self-management support
  • Deliver high quality clinical assessment, care planning and ongoing management
  • Collaborate effectively within a multidisciplinary team
  • Participate in service development, quality improvement initiatives and clinical audits to enhance patient outcomes
  • Maintain accurate and timely clinical documentation in accordance to organisational and legal requirements
  • Engage is ongoing professional development to maintain credentialing and contemporary clinical knowledge

 

Skills & Experience

  • Experience in diabetes education is highly desirable; Credentialled Diabetes Educator (CDE) status highly regarded.
