About the Role
Are you an experienced and passionate nurse with expertise in dementia, memory support, and community-based care? Grampians Health is seeking a Clinical Nurse Consultant (CNC) - Memory Nurse to provide specialist support to older people living in the community who are experiencing mild cognitive impairment (MCI), dementia, other cognitive changes impacting function, and chronic and complex health needs.
This rewarding role combines advanced clinical practice, leadership, education, and service development to support clients and their carers to maintain independence, wellbeing, and quality of life.
Key Responsibilities

Deliver person-centred nursing assessments and interventions for older people with dementia and complex needs.

Provide advanced clinical decision-making, assessment and care planning to support client wellbeing.

Educate, mentor and support nursing staff in dementia care and behavioural symptom management.

Collaborate with multidisciplinary teams and contribute to case conferences and integrated care planning.

Facilitate timely referrals, future planning and access to appropriate support services.

Promote wellness, reablement and evidence-based non-pharmacological interventions.
Skills & Experience
-
Registered Nurse Div 1 with current AHPRA registration.

Postgraduate qualification in a relevant health field.

Experience supporting older people, dementia and complex care needs.

Strong assessment, clinical reasoning and multidisciplinary teamwork skills.

Current Victorian Driver's Licence and sound computer literacy

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant A (ZF4) which is $61.58 per hour.
