About the Role
Fancy a secondment from your current role or keen to add new skills to your CV? Grampians Health is seeking a Clinical Nurse Consultant to join the Residential In Reach (RIR) team. This nurse led multidisciplinary service provides expert, person-centred care to residents in public and private aged care facilities across the Grampians region.
You’ll work independently to prevent unnecessary hospital admissions, facilitate early discharge, and support aged care staff with education and clinical guidance. You’ll collaborate closely with Nurse Practitioners, GPs, geriatricians, ward staff, and emergency departments to deliver safe, coordinated care aligned with residents’ wishes and the Strengthened Aged Care Quality Standards.
Key Responsibilities
• Provide timely nursing assessments and develop individualised treatment plans
• Support aged care homes during infectious disease outbreaks
• Deliver education and capacity building to aged care staff
• Facilitate early discharge and prevent unnecessary hospital admissions
• Liaise with ED, inpatient units, and GPs to coordinate care
• Contribute to service redesign and regional RIR project delivery
Skills & Experience
• Registered Nurse with AHPRA and current practising certificate
• Strong clinical assessment, decision-making, and acute nursing skills
• Proficient in IV therapy, ECG, IDC management, and general acute nursing care
• Postgraduate qualification in aged care or related field preferred
• Excellent communication and stakeholder engagement skills
