About the Role

The Grampians Transition Care Program (TCP) facilitates the provision of 63 Commonwealth and State funded bed-based (residential) and home-based (community) places across the Grampians region. TCP is a goal-oriented, time-limited program supporting older people following a hospital episode to optimise functional capacity and transition to longer-term care arrangements.

 

The TCP Intake and Triage Coordinator, in collaboration with the TCP Team Leader, is responsible for managing referrals to the program in accordance with the Transition Care Program Guidelines 2026. This includes triage, screening, liaison with referrers and Aged Care Assessment Services, and coordinating entry to the program including consent and client agreement processes.

 

Key Responsibilities

  • Coordinate and manage TCP referrals, triage and admissions in line with program guidelines

  • Collaborate with internal and external stakeholders to support timely patient access

  • Provide clinical guidance to promote a consumer-centred model of care

  • Support regional bed management to maintain occupancy and service efficiency

  • Ensure accurate documentation aligned with TCP Guidelines and Aged Care Quality Standards

  • Contribute to continuous improvement, quality activities and multidisciplinary teamwork

 

Skills & Experience

  • Tertiary qualification in Nursing or Allied Health with current AHPRA registration

  • Demonstrated experience in assessment, care planning and discharge planning for complex patients

  • Strong understanding of TCP principles and aged care frameworks

  • Advanced clinical reasoning, communication and stakeholder engagement skills

  • Experience working within multidisciplinary and/or community-based environments

  • High level computer literacy and ability to manage competing priorities
