Who are we?
Aged Care Assessment - Grampians (part of Grampians Health) is a well-regarded aged care assessment organisation, that provides community and hospital based aged care assessment across the Grampians Region.
About the Role
The purpose of this role is to undertake personal Home Support assessments for My Aged Care clients. The Home Support assessment is designed for people who may need entry-level support to help them stay at home. Non-Clinical Assessors incorporate the Aged Care Assessment Quality framework to deliver high quality assessments, with a focus on client experience. The aim is to work with clients to develop support plans to achieve wellness and healthy ageing goals through linking and referral to the client's chosen support services through the My Aged Care Gateway.
Key Responsibilities
- Complete Home Support Assessments using the Integrated Assessment Tool (IAT) in line with program guidelines, timeframes and quality requirements using the standardised Aged Care Assessment Quality Framework.
- In completing the Aged Care Assessment, follow the principles in the My Aged Care Assessment Manual.
- Work as part of the Aged Care Assessment - Grampians Team to coordinate client referrals and link with appropriate service providers.
- Support the delivery of coordinated and transparent services through the centralised client record and My Aged Care Gateway.
- Working remotely within a region where you will be required to travel long distances between clients.
Skills & Experience
- Certificate IV or Diploma relevant to the aged care sector, or significant progress towards such a qualification.
- Demonstrated knowledge and/or experience in working with people who are aging and their families/carers.
- Demonstrated understanding/experience of the provision of community-based support services to the aged which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processes.
- Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach.
