About the Role

Grampians Palliative Care (GPC) is seeking an enthusiastic and dedicated Nurse Practitioner to join our regional consultancy team to cover parental leave in Ballarat. This role is integral to providing specialist palliative care support to patients and their families across the entire Grampians region.  Nurse Practitioners at GPC are key members of the interdisciplinary team and work closely with palliative care Physicians, Clinical Nurse Consultants, Allied Health professionals, community-based palliative care teams, cancer care and non-malignant palliative care Coordinators across the region.

As a service, GPC aims to:

  • contribute to high-level palliative care delivery across the Grampians region 
  • facilitate the ongoing service development of palliative care services in the Grampians region, ensuring equitable access to high-quality, inclusive, consumer-focused, palliative care services
  • promote awareness of palliative care services and practices among regional health providers
  • identify and address the educational needs of health professionals to ensure effective and appropriate palliative care delivery
  • promote awareness of palliative care Standards
  • identify strategies to ensure appropriate support and debriefing for palliative care providers

 

Key Responsibilities:

  • assist in the ongoing development, delivery, and evaluation of all aspects of specialist palliative care services across the Grampians region.  This includes comprehensive assessment and management of complex palliative care patients and their families within the scope of a palliative care Nurse Practitioner and Nurse Practitioner Standards of Practice.
  • support acute and sub-acute inpatient services and community clients including in Residential Aged Care
  • engage in education, research, leadership, and clinical consultation to enhance palliative care services for the region

 

Skills & Experience

  • AHPRA Registered Nurse registration with Nurse Practitioner endorsement with relevant experience in palliative care
  • Commitment to apply for all relevant credentialing requirements, i.e. provider and prescriber numbers relevant to GH
  • Demonstrated ability to work independently, as well as collaboratively within the interdisciplinary team
  • Strong communication and leadership skills
  • Commitment to ongoing professional development and education in palliative care
  • Current drivers licence
