40 hours per week including a monthly ADO | Fixed Term until June 2027
About the Role
This Occupational Therapist Grade 2 position sits within the Community Programs team, delivering services as part of the Hospital Admission Risk Program (HARP). The role focuses on supporting clients with complex and chronic conditions to remain safely in their home environment, reducing avoidable hospital admissions through targeted, evidence-based interventions. You will work as part of a multidisciplinary team to provide person centred care that promotes independence, safety, and quality of life.
The role also includes a rotating caseload across community programs at Grampians Health, including the Commonwealth Home Support Program (CHSP), Home and Community Care (HACC), Care Communities (Residential Aged Care), and Linkages (Home Care Packages). This provides the opportunity to work across a range of settings and client groups, building broad skills in community and aged care while responding to diverse and evolving client needs.
You will be supported by experienced senior Occupational Therapy staff, with access to supervision, in-service education, and ongoing professional development within a collaborative and well supported team environment.
Key Responsibilities
• Assess function, home safety and support needs
• Plan and deliver targeted OT interventions
• Prescribe assistive tech and equipment
• Liaise with families, carers and funding bodies
• Supervise students and support junior staff
• Maintain accurate clinical documentation
Skills & Experience
• Degree in Occupational Therapy
• AHPRA registration (or eligible)
• Experience in community or aged care settings
• Skills in assistive tech and funding pathways
• Ability to supervise students or staff
