Grade 2 Physiotherapist – Transition Care Program
Fixed Term 1.5 days/week – 19/01/26 to 31/01/27
About the Role
We are seeking a motivated and skilled Physiotherapist to join our Transition Care Program (TCP) team. This program supports older adults as they transition from hospital to home or residential care, focusing on rehabilitation and independence.
Key Responsibilities:
- Deliver evidence-based physiotherapy interventions to optimise mobility and functional outcomes.
- Conduct comprehensive assessments and develop individualised care plans.
- Work collaboratively within a multidisciplinary team to achieve client goals.
- Provide education and support to clients, families, and carers.
- Assist with discharge planning and liaise with community services.
About You
- Current AHPRA registration as a Physiotherapist.
- Experience in geriatric rehabilitation, subacute care, or community health (preferred).

- Demonstrate an understanding of relevant service development activities, including evaluation, monitoring, clinical governance and quality improvement.
- Strong communication and teamwork skills.

- Valid driver’s license (community visits may be required).
