About the Role

Join the Hospital Admission Risk Program (HARP), a community-based service supporting people with chronic conditions, ageing-related needs, and complex care requirements. This role offers strong autonomy and clinical ownership, working across hospital and community settings to reduce avoidable admissions and improve patient outcomes. You’ll act as a central coordinator, delivering person-centred care and partnering with multidisciplinary teams.

• Deliver short-term, goal-focused care coordination
• Act as key contact for clients, carers, and providers
• Support self-management through education
• Complete client-centred assessment and care planning
• Collaborate across hospital and community services

 

Key Responsibilities

• Assess clients and develop goal-based care plans
• Coordinate care across multidisciplinary teams
• Provide education to support self-management
• Advocate for clients and carers
• Plan discharge and transition of care
• Deliver HARP in line with HIP guidelines

 

Skills & Experience

• Registered Nurse (Div 1) with AHPRA registration
• Postgrad study in chronic disease, MH, AOD or similar
• Experience in complex or community-based care
• Strong assessment, planning and communication skills
• Current driver’s licence

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant (ZF4-ZJ4) which ranges from $59.79 - $62.94 per hour.
