Part time| Fixed Term until mid‑July 2027 (Maternity Backfill position)
A rewarding opportunity is available for a motivated and skilled Grade 1 or Grade 2 Social Worker to join our team in a part-time (0.7 EFT), fixed-term role until July 2027. Based in the BRICC building at Ballarat Base Hospital, you will play a key role in supporting vulnerable clients within a specialist multidisciplinary environment.
This role sits within a specialised social work team providing services across chronic illness, cancer care, ICU, care communities, and inpatient settings. The successful applicant will bring strong organisational skills, sound clinical reasoning, and well-developed competencies in psychosocial assessment, intervention planning, and collaborative team practice.
This position provides an excellent career pathway for a capable and ambitious Grade 1 Social Worker or a skilled Grade 2 Social Worker seeking to further develop their practice.
Social work opportunities at this level are rare within our service, offering the chance to broaden practice, build confidence, and strengthen clinical decision-making in a supportive and dynamic environment.
Key Responsibilities
As a Grade 1 or 2 Social Worker, you will:
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Undertake psychosocial and risk assessments (family violence, elder abuse and child safety)
- Complex discharge planning
- Communicate with a wide range of professionals
- Collaborate in pursuit of patient and team needs/goals
