About the Role
Are you a dedicated Social Worker Grade 2 seeking a new and rewarding challenge? Grampians Health Ballarat is looking for a passionate Social Worker to join our forward thinking team. Due to internal promotion, a role has become vacant in the highly sought after Women’s and Children’s team, but the role also offers the opportunity to work across a range of services, including acute care, outpatient services, inpatient rehabilitation, complex care, and community rehabilitation.
As part of our Social Work team, you will play a vital role in enhancing the psychological and social wellbeing of our patients. You will ensure that each patient’s social, emotional, and cultural needs are met, while working to minimise the impact of illness and hospitalisation. Key responsibilities include conducting psychosocial and risk assessments, facilitating complex discharge planning, and providing support to both patients and their families.

Key Responsibilities
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Undertake psychosocial and risk assessments (family violence, elder abuse and child safety)
- Complex discharge planning
- Communicate with a wide range of professionals
- Collaborate in pursuit of patient and team needs/goals
Skills and experience
- Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree)
- Demonstrated ability to monitor, evaluate and modify own performance through reflective practice
- Ability to adapt, be flexible and learn quickly
- Work well under pressure
- Ability to plan and problem solve
- Demonstrated knowledge in Social Work theory and practice skills
- Some knowledge of working in acute/sub-acute environments is desirable
- Ability to provide Social Work supervision or work toward this skillset
- Diverse Experience: Opportunities to rotate caseloads, gain hands on experience in orthopaedics, geriatrics, neurology, oncology, general medicine, surgery and paediatrics
