About the Role

Are you a dedicated Social Worker Grade 2 looking for a new and rewarding challenge? Grampians Health Ballarat is seeking a passionate and motivated Social Worker to join our progressive and highly regarded Social Work team. As a Grade 2 Social Worker, you will have opportunities to contribute across a broad spectrum of services, including:

  • Acute Care
  • Outpatient Services
  • Inpatient Rehabilitation
  • Complex Care
  • Community Rehabilitation

 

Key Responsibilities

  • Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families 
  • Undertake psychosocial and risk assessments (family violence, elder abuse and child safety) 
  • Complex discharge planning 
  • Communicate with a wide range of professionals 
  • Collaborate in pursuit of patient and team needs/goals 

 

Skills and experience 

  • Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree) 
  • Demonstrated ability to monitor, evaluate and modify own performance through reflective practice 
  • Quick learner with strong adaptability, planning, and problem-solving skill
  • Demonstrated knowledge in Social Work theory and practice skills 
  • Some knowledge of working in acute/sub-acute environments is desirable 
  • Ability to provide Social Work supervision or work toward this skillset 
  • Diverse Experience: Opportunities to rotate caseloads, gain hands on experience in orthopaedics, geriatrics, neurology, oncology, general medicine, surgery and paediatrics
