About the Role
The Post Acute Care (PAC) Program provides short-term services and support for patients who need extra help at home after a public hospital stay. The Team Leader, Post Acute Care, will lead the Post Acute Care team in coordinating the delivery and continuity of services for the patient/client following an acute or subacute admission. The role will work closely with range of clinicians and health care professionals, internally and externally, to complement and improve outcomes of care, systems, and services.
Key Responsibilities
- Development and review of referral pathways across the health system as required.
- Lead and support the PAC/Non-admitted stream staff to deliver high quality, coordinated patient/client journeys
- Monitor implementation of care against the patient/client care plan in collaboration with the clinical care team.
- Promote a positive team culture and foster a collaborative, supportive environment that encourages creativity, innovation, and continuous improvement.
- Facilitate and contribute to team meetings, collaborate with colleagues to solve challenges as they arise
- Assist the Manager, Hospital Without Walls, in achieving financial and operational objectives by monitoring team performance, supporting the achievement of activity targets
- Ensure a collaborative working relationship with the Manager and other Hospitals Without Walls Clinical Navigators in relation to decision making
- Provide expert clinical guidance and support to team members to ensure clinical standards, policies, and procedures are consistently followed
- Role model positive communication and decision-making skills within the team, ensuring clarity in roles, responsibilities, and expectations
- Ensure compliance with clinical governance, risk management, and safety frameworks including NSQHS Standards, AHPRA standards, and organisational policies
- Lead or support change management initiatives to improve service delivery, patient flow, or integration across the care continuum
Qualifications & Experience
- Registered Nurse with current Australian Health Practitioner Regulation Agency (AHPRA) registration.
- Minimum five years post-registration nursing experience in diverse healthcare settings (acute, sub-acute, or community).
- Demonstrated experience and competence in clinical governance, quality improvement, risk management, and accreditation/compliance processes
- Knowledge of regional after-hospital and community services.
- Demonstrated supervisory/leadership experience (NUM/Team Leader) in post-acute care, community nursing, discharge planning, sub-acute or integrated care (desirable)
- Post-graduate qualification (or working toward) in nursing, health services management, community health or related field (desirable).
- Experience with virtual care, telehealth, or remote-monitoring technologies (desirable).
- Proficiency in relevant systems (e.g., OPAL, UNITI, RIMS, Microsoft Teams) and understanding of healthcare funding streams (desirable).
