Fixed Term Part Time position – 0.9 FTE | Possibility for extension of fixed term | Location - Horsham, Stawell, Dimboola, Edenhope

 

About the Role

Due to significant and rapid expansion of the Residential in Reach (RIR) service, an opportunity exists for a skilled Clinical Nurse Consultant to join the team on a fixed term basis, with potential for extension. Based in the Grampians Health Western region, you will work autonomously to prevent avoidable hospital admissions, support early discharge and strengthen clinical capability within residential aged care.

You will partner with older people, Nurse Practitioners, GPs, Geriatricians, ward teams, aged care staff and Emergency Departments to deliver safe, coordinated care aligned with residents’ goals and the Strengthened Aged Care Quality Standards.

 

Key Responsibilities

  • Provide timely nursing assessment and care plan
  • Support aged care homes during outbreaks
  • Deliver education and build staff capability
  • Facilitate early discharge and admission avoidance
  • Liaise with ED, wards and GPs to coordinate care
  • Contribute to RIR service redesign and projects

 

Skills & Experience

  • Registered Nurse with AHPRA registration
  • Strong acute assessment and clinical decision skills
  • Skilled in IV therapy, ECG and IDC management
  • Postgraduate study in aged care preferred
  • Effective communication and stakeholder skills
  • Minimum 5 years recent acute experience
