About the Role

The Post Acute Care (PAC) Coordinator is responsible for coordinating short-term support services for patients discharged from Grampians Health and other public hospitals. The role delivers up to 28 days of funded care, including personal, nursing, domestic, and community assistance, to support recovery and independence at home.

Aligned with the Hospital Without Walls framework, this position ensures seamless care transitions between acute, subacute, and community settings. Through collaboration with multidisciplinary teams and community providers, the PAC Coordinator strengthens care pathways and promotes high-quality, person-centred care across the region.

 

Key Responsibilities

  • Conduct intake, screening, and assessment processes to ensure safe and timely discharges.
  • Develop and review individualised care plans in partnership with patients, families, and providers.
  • Coordinate and monitor the delivery of in-house and external support services.
  • Provide clinical guidance and support to PAC Health Care Workers.
  • Facilitate referrals and smooth transitions to ongoing community-based care.
  • Maintain accurate documentation and uphold professional, person-centred standards in all interactions.

 

Qualifications & Experience

  • Registered Nurse with current AHPRA registration and practising certificate.
  • Strong experience in acute, subacute, or community settings.
  • Demonstrated advanced assessment, clinical reasoning, and care coordination skills.
  • Proven ability to manage competing priorities and perform under pressure.
  • Strong interpersonal and communication skills to build effective professional relationships.
  • Proficiency in relevant systems (e.g., OPAL, UNITI, RIMS, Microsoft Teams) and understanding of healthcare funding streams (desirable).
