About the Role
This newly developed role is a unique opportunity to provide high quality, patient-centred social work support within the Emergency Department at Grampians Health Horsham. Working as part of a supportive team of more than 40 social workers, you will deliver psychosocial and risk assessments, crisis intervention, counselling and care planning for patients experiencing complex needs including AOD concerns, family violence, child protection, elder abuse and homelessness.
The role also supports acute inpatient wards and contributes to supervision, education and service development activities across the wider team. It is expected that you will have a strong working knowledge of the AOD support and referral pathways, as well as Child Protection and Family Violence response system (MARAM alignment).
Key Responsibilities

Manage clinical caseloads in the Emergency Department

Complete assessments, care plans and documentation

Provide grief and bereavement support

Maintain effective multidisciplinary communication

Support Grade 1 staff and student supervision

Deliver education to regional urgent care teams
Skills & Experience
-
Degree in Social Work with AASW eligibility
-
Current Victorian Driver’s Licence
-
Experience in acute health or community services
-
Knowledge of AOD, Child Protection and MARAM
-
Strong communication and conflict resolution skills
