About the role
Join a dynamic and supportive Social Work team that values compassion, learning, and collaboration. Based at our Horsham campus, this Grade 2 role supports patients primarily in the ward setting, with a small amount of service to outpatients and residents in our GH aged care facilities. You will help people improve their health and wellbeing by addressing the psychosocial factors that affect their lives.
You’ll be part of a broader network of over 40 Social Workers across Grampians Health, and have access to a range of structured supports including in-service training, Grade 2 supervision, external professional development, and study leave. Key responsibilities

Deliver Social Work services across acute, subacute and community settings

Complete psychosocial and risk assessments and care planning for patients

Provide grief and bereavement support to patients and families

Liaise effectively with internal teams and external community agencies

Participate in clinical supervision, service development, and quality activities

Support clients and teams with informed decision-making and advocacy

Provide psycho-education

Counselling and crisis intervention
Skills and experience
-
Eligible for AASW membership (Bachelor or qualifying Master’s in Social Work)

Demonstrated knowledge of psychosocia/risk assessment and care planning

Strong communication and interpersonal skills
-
Ability to work independently and as part of a multidisciplinary team

Understanding of hospital systems and community resources
