Permanent Full Time (with monthly ADO) | HARP and Community Health

About the role

Join a compassionate and collaborative Social Work team across our Stawell and Horsham campuses. This Grade 2 role primarily supports patients in Community Health and the Hospital Admission Risk Program (HARP), with some involvement in inpatient wards and aged care services. Working 2 days per week in Horsham and 3 days in Stawell, you will help improve health and wellbeing by addressing the psychosocial factors impacting people’s lives.

As part of a network of more than 40 Social Workers across Grampians Health, you'll benefit from strong professional support, including supervision, in-service training, professional development opportunities, and study leave. This is an excellent opportunity for a social worker passionate about person-centred care and professional growth in the health sector.

Key responsibilities

• Deliver Social Work services in the community setting and in people's homes

• Complete psychosocial and risk assessments and care planning for patients

• Provide grief and bereavement support to patients and families

• Liaise effectively with internal teams and external community agencies

• Participate in clinical supervision, service development, and quality activities

• Support clients and teams with informed decision-making and advocacy

• Provide psycho-education

• Counselling and crisis intervention

Skills and experience

• Eligible for AASW membership (Bachelor or qualifying Master’s in Social Work)

• Demonstrated knowledge of psychosocial/risk assessment and care planning

• Strong communication and interpersonal skills

• Ability to work independently and as part of a multidisciplinary team

• Understanding of hospital systems and community resources
