About the Role
An exciting fixed‑term opportunity exists for an experienced nurse or allied health professional to step into the Stroke Co‑ordinator role at the Horsham campus. This role is central to ensuring high‑quality, coordinated and person‑centred stroke and TIA care from emergency presentation through acute, rehabilitation and palliative pathways.
You’ll provide expert guidance to multidisciplinary teams, support delivery of stroke unit standards, and champion best‑practice stroke care aligned with the Acute Stroke Service framework and rehabilitation guidelines. The role combines advanced clinical assessment, education, coordination and data reporting to improve outcomes for people impacted by stroke.
What you’ll do
- Provide expert advice on confirmed or suspected stroke/TIA care
- Support coordinated inpatient stroke care across the full care pathway
- Champion best‑practice, evidence‑based stroke standards
- Lead stroke education and workforce upskilling
- Coordinate referrals, discharge planning and follow‑up care
Skills & Experience
- Registered Nurse or Allied Health professional with current AHPRA registration
- Strong experience in acute or sub‑acute stroke care
- Advanced clinical assessment and decision‑making skills
- Ability to work autonomously and within multidisciplinary teams
- Well‑developed communication, organisation and problem‑solving skills
