VAD Regional Care Coordinator

Permanent Part Time | 8 hours per week, flexible days of the week
We are seeking a health professional/s (CNC, Nursing or Allied Health) interested in a part time job share based in Horsham as the Western Regional Grampians VAD Navigator.
You will be supported by undertaking specific training, on the job orientation and will have the support of a small team of professionals based in both the Ballarat Base Hospital and from the Statewide Navigator service based in Peter Macallum hospital, Melbourne.
About th Role
Voluntary assisted dying care navigators are a point of contact for the community, health practitioners and health services across the Victoria who seek information about or assistance with voluntary assisted dying. The Grampians Regional VAD Navigator is a department of health funded position, hosted and supported by Grampians Health and supported by the Statewide Navigator service based in Pater MacCallum, Melbourne.
VAD navigators work closely with the person, their carers, family and friends, medical practitioners and healthcare teams to tailor support that meets the needs of the person with part of their end-of-life planning.
Key Responsibilities
- Providing general information about voluntary assisted dying in Victoria
- Individualised support and information, either in a face-to-face consultation or by Australian Post
- Assistance in connecting people with appropriate medical practitioners and health services
- Information about or access to voluntary assisted dying support packages
- Holistic advice and follow-up on appropriate end-of-life care services
- Education to health services and practitioners.
The successful applicant will work with the Ballarat based VAD Navigator to provide cover to the Western Grampians region and will work collaboratively and consultatively with key medical practitioners and health care providers across the Grampians region to provide VAD related information and coordination to the community within the confines of the State VAD legislation.
Skills & Experience
- Experienced in direct patient care within a health-related field.
