About the Role
Join a dynamic community health team supporting referral intake and coordination across multiple programs. This role offers variety and autonomy while providing the opportunity to make a meaningful impact by connecting people to essential care services.
You will work across multiple systems to ensure accurate data entry, timely triage of referrals, and responsive, client-centred service delivery, while supporting both community members and internal teams.
Key Responsibilities
• Triage referrals and assign priority of access
• Enter and maintain client records in UNITI
• Provide telephone support and intake assistance
• Assist with My Aged Care referral processes
• Support reporting across HACC, CHSP and DEX
• Liaise with internal teams and external providers
Skills & Experience
• Cert III Business Admin or relevant experience
• Strong admin skills in health/community sector
• Proficient in MS Office and client systems
• Knowledge of My Aged Care & referral pathways
• Current Police, WWCC, NDIS check (or willingness to obtain)
