About the Role

The Transition Care Program (TCP) is a goals-based program and provides short term care coordination, allied health and support services to older people following a hospital admission. The TCP Care Coordinator utilises a client centred approach to ensure collaborative care planning and monitoring, service facilitation and continuous communication with clients and their family/care providers. TCP can be provided in a bed-based or home-based setting, depending on client goals and needs.  Clients may move between these settings during the TCP episode.

 

 

Key Responsibilities

 

  • Provide short term support and active care coordination for older in line with the TCP Programme Guidelines.
  • Coordinate all aspects of client care including admissions, care planning, care monitoring and exit/discharge planning.
  • Ensuring a multi-disciplinary approach
  • Liaising and developing positive working relationships with internal and external stakeholders including the hospital team, GP’s, Geriatricians and other specialists, Aged Care Assessment

 

 

Skills & Experience including

  • Registered Nurse or Allied Health Professional with a current AHPRA registration.
  • Experience working in a community setting, hospital environment or aged care setting with development of relevant clinical knowledge, clinical and problem solving skills.
  • A current Australian Driver’s License.
