About the Role

The Clinical Intake Worker supports access, coordination, and the client journey across community and non-admitted Allied Health and Ambulatory Care services at Grampians Health Stawell.

This role is part of a central intake team, working alongside administration service access staff to ensure timely, appropriate, and coordinated access to care.

 

Key Responsibilities

  • Acting as an initial point of contact for clients
  • Patient registration and information gathering
  • Initial needs identification
  • Clinical screening and clinical triage
  • Liaising with internal and external providers

 

We are seeking applications from clinicians interested in non–hands-on clinical work, who are motivated to use their clinical knowledge in an access and triage-focused role. The position provides exposure to multiple disciplines and requires regular engagement with key stakeholders across those services.

The successful applicant will have excellent communication skills and a strong commitment to supporting patients to access the right care, at the right time, in the right location.

 

You will bring:

  • Strong skills in medical terminology
  • Knowledge of public health pathways and local agencies
  • The ability to use clear, lay language to sensitively ask prompting questions about a person’s health and social needs when accessing services

 

The successful applicant will join the Ambulatory Care Team at Grampians Health Stawell, contributing to a patient-centred, coordinated model of care.
