About the Role
As a District Nurse, you’ll provide essential community-based clinical nursing services, promoting safe, effective, and consumer-centred care. Working within a multidisciplinary team, you'll support consumers in achieving their health goals. You’ll also contribute to social support programs aimed at helping individuals remain independent, socially engaged, and active in their communities.
This role involves delivering services through programs such as Commonwealth Home Support Program (CHSP), Support at Home, Post-Acute Care (PAC), Hospital in the Home (HITH), Home and Community Care Program (HACCPYP), and National Disability Insurance Scheme (NDIS) and a number of private brokerage contracts.
Key Responsibilities
- Plan and evaluate nursing care in collaboration with consumers, carers, families, and care teams.
- Conduct thorough nursing assessments of consumers and their environments.
- With consumer consent, communicate and collaborate effectively with others to facilitate the provision of goal directed care plans.
- Deliver safe, effective, and evidence-based care to meet consumer goals.
- Respond swiftly and effectively to changing clinical conditions or consumer deterioration.
- Engage in continuous professional development and proactively identify personal learning needs.
Skills & Experience
- Medication Endorsed Enrolled Nurse with current AHPRA registration.
- A current Australian Driver's License.
- Experience working in a community setting, hospital environment or acute settings where advanced achievement in a range of clinical skills such as wound care, continence care and diabetes management can be demonstrated.
- Demonstrate advanced assessment and consumer person-centred care planning skills in keeping with wellness and re-enablement principles of care.
