About the Role

Join our community-based District Nursing team and support consumers to achieve their health goals at home. This role delivers holistic, consumer-centred nursing care across a range of funded community programs. You’ll work autonomously and collaboratively to provide safe, effective and evidence-based care that makes a real difference.

 

Core duties

• Deliver community-based nursing care
• Support consumer health goals at home
• Work across funded care programs
• Collaborate within multidisciplinary teams
• Promote safe, high-quality practice

 

Key Responsibilities

• Assess, plan and deliver evidence-based nursing care
• Coordinate care with consumers, carers and teams
• Respond to changing clinical needs and priorities
• Maintain accurate clinical documentation
• Uphold infection control and safety standards
• Participate in quality improvement activities

 

Skills & Experience

• Registered Nurse with current AHPRA registration
• Advanced clinical skills in community or acute care
• Strong assessment and care planning capability
• Effective communication and teamwork skills
• Current Australian Driver’s Licence

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is District Nurse Level 2.3 (YQ3)  to District Nurse Level 2 (YU1) which ranges from $50.76 - $55.07 per hour.
