About the role
Join a dynamic and supportive Social Work team that values compassion, learning, and collaboration. Based at our Stawell campus, this Grade 2 role supports patients primarily in the community health outpatient setting, with a small amount of service to inpatient wards and residents in our GH aged care facilities. You will help people improve their health and wellbeing by addressing the psychosocial factors that affect their lives. You will be skilled in:
 
You’ll be part of a broader network of over 40 Social Workers across Grampians Health, and have access to a range of structured supports including in-service training, Grade 2 supervision, external professional development, and study leave. Key responsibilities
  • Deliver Social Work services across acute, subacute and community settings
  • Complete psychosocial and risk assessments and care planning for patients
  • Provide grief and bereavement support to patients and families
  • Liaise effectively with internal teams and external community agencies
  • Participate in clinical supervision, service development, and quality activities
  • Support clients and teams with informed decision-making and advocacy
  • Provide psycho-education
  • Counselling and crisis intervention
 
Skills and experience
  • Eligible for AASW membership (Bachelor or qualifying Master’s in Social Work)
  • Demonstrated knowledge of psychosocia/risk assessment and care planning
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Understanding of hospital systems and community resources
